The form is at http://www.contrabass.com/pages/survey.html. I ask for name and email address, just to make sure I don't double-count anyone (a fair number of results come in duplicate or triplicate). Yours isn't in the list, so please feel free to post one :-)

BTW, if anyone is interested in a decent resin bass clarinet, I've just posted my Yamaha YCL-220 for sale at eBay (http://cgi.ebay.com/aw-cgi/eBayISAPI.dll?ViewItem&item=307428651). It's a good horn for students or doublers (I originally bought it for doubling in a jazz ensemble), but I've recently upgraded to a wooden Selmer for use in the SJWS. Good low register (to low Eb), and has an altissimo vent. The Yamaha is barely used, and even lacks the traditional huge dent in the bottom of the bell ;-)
